COVID-19 Vaccine Intake Questionnaire Instructions

To complete your COVID-19 Vaccine Intake Questionnaire, follow the instructions below:

• Log into your Student Health Portal
• Use your Mercy credentials to log into the portal.
• In the blue menu banner, go to "My Forms"
   
• Find the "COVID-19 Vaccine Intake Questionnaire" to fill out
• Click on the questionnaire and submit

If you wish to pursue an exemption for medical reasons or religious/spiritual beliefs, or if you are unvaccinated and will be a fully remote Distance Learning student, please reach out the Office of ACCESSibility (accessibility@mercy.edu) to receive thee appropriate forms. Supporting documentation will be required.

ALL EXEMPTION REQUESTS MUST BE RECEIVED BY AUGUST 1 , 2021
